  • Supported Living (SL)

    Our Supported Living services empower individuals to live in their own homes with the assistance they need to succeed. Whether it’s help with daily living tasks, managing finances, or fostering community connections, we work side by side to ensure our clients maintain independence and dignity.

  • Independent Living Services (ILS) and Individualized Living Training (ILT)

    ILS/ILT focuses on building essential skills for independent living. From meal planning and budgeting to navigating public transportation, our team offers hands-on training tailored to each client’s goals, enabling them to live more independently with confidence.

  • Coordinated Family Services (CFS)

    CFS is designed for individuals who choose to live at home with their families. We provide tailored support to help individuals and their loved ones access community resources, coordinate care, and achieve their personal goals in a way that respects their cultural and familial values.

  • Personal Assistance

    For individuals needing extra help with day-to-day activities, our Personal Assistance services offer compassionate, one-on-one support. We prioritize respect, dignity, and the unique needs of every individual, ensuring they feel valued and supported.

  • Tailored Day Program

    Our Tailored Day Program offers a flexible approach to day services, designed to fit the goals and aspirations of each participant. Whether pursuing education, employment, or community involvement, we work together to create a day that is both productive and fulfilling.

  • Social Recreation

    Our Social Recreation program prioritizes designing activities based on the input and feedback of our participants. This approach ensures we address the unique skills and resources needed to help individuals independently access social and recreational opportunities in their communities. Through activities such as board games, arts and crafts, and outings, participants develop confidence, refine social skills, and build meaningful connections. By fostering independence, we empower participants to bridge gaps in accessing inclusive and enriching community experiences.

  • Community Based Day Program

    Civitas’ Community-Based Day Program supports adults with developmental disabilities in building life skills, developing work readiness, and engaging in meaningful volunteer experiences—all within small group settings. Our program is person-centered, offering hands-on opportunities to explore community resources, practice communication and social skills, and gain confidence through real-world experiences. Participants engage in activities such as volunteering at local organizations, learning daily living tasks, exploring career interests with staff support, and learning to navigate public transportation. The ultimate goal is to foster greater independence, encourage purpose-driven community involvement, and prepare individuals for employment and self-reliant living.
