Supported Living Coordinator
Responsible for ensuring that individualized services are delivered with excellence, dignity, and effectiveness. Hire, train, and supervise direct support staff. Coordinate client care and implement Individual Service Plans across residential settings, including visiting the residences (located throughout San Diego County), overseeing day-to-day care, and ensuring clients’ needs are met. Maintain schedules, documentation, and compliance with service standards. Monitor medication management and write incident reports as needed. Support skill-building in daily living activities. Participate in the Manager-on-Call rotation, during some evening hours and/or weekends. Ensure client safety and provide emergency/crisis support when necessary.
Position Details
JOB DUTIES
Bachelor’s degree or foreign equivalent in social science, psychology, or related field. Two (2) years of experience in job offered, direct support services, or a related occupation. In lieu of a bachelor’s degree and 2 years of experience, employer will accept four (4) years of experience in job offered direct support services, or related field. Must have valid driver’s license, auto insurance, and own vehicle to visit client residences located throughout San Diego County. Any amount of experience in additional requirements section below will be acceptable unless specific amount is noted. All experience can be concurrent. Additional requirements: Prior academic background or work experience to include: 1) At least two (2) years’ experience working with people with developmental, intellectual, or mental disabilities, creating and implementing person - centered support that fosters independence, dignity, safety, and inclusion. 2) At least two (2) years’ experience communicating individualized care and support plans with clients, care givers, and support staff. 3) Experience drafting and maintaining care documentation, including documenting plans, goals, medications, progress, and incidents. 4) Experience with Office365. Ability to support Manager-On-Call rotations during evening hours and weekends.

$62,296 to $72,296 per year
